Please note that if you file your cost report using the CMS Free software you must also submit a hard copy of the entire cost report as the free software does not create a complete copy. Failure to do so will cause your cost report to be rejected. To access the hard copy forms as well as the instructions for filling out the cost report, please use the following link:

Once you click on this link you will be taken to the CMS website and CMS Pub. 15-II. On the website you will need to choose the chapter that applies to your cost report:
SNF = Chapter 35
HHA = Chapter 32
RHC = Chapter 29
OPO = Chapter 33
ESRD = Chapter 34
Hospice = Chapter 38
CMHC/CORF/OPT = Chapter 18
Once you click on the chapter that applies to your cost report you will be instructed to open a zip file or save it. Inside of this zip file are the instructions and the forms themselves. |
